Superhuman Performance Medicine – Client Policies & Consent
1. Acceptance of Terms
By accessing or using services provided by Superhuman Entrepreneur DBA Superhuman Performance Medicine (“SPM,” “we,” “us”), you acknowledge that you have read, understood, and agree to these policies, along with our Terms of Service and Privacy Policy.
2. Nature of Services
SPM provides a combination of:
Health coaching, nutrition, and wellness services (non-medical)
Access to licensed healthcare providers for evaluation, lab review, and treatment when appropriate
Health coaching services are not a substitute for medical care. Any medical services are provided solely by licensed healthcare providers and are subject to their independent clinical judgment.
Our services are not intended for emergency care. If you are experiencing a medical emergency, call 9-1-1 immediately.
3. Laboratory Testing & Results
Laboratory testing may be recommended based on your intake and consultations.
Payment is required at the time of purchase
We do not accept insurance
Results are typically available within 2–3 weeks, though timing may vary
Once results are available:
You will be scheduled for a virtual consultation
You will receive a copy of your results electronically
4. Outcomes & Adherence
You may receive recommendations including:
Nutrition and lifestyle changes
Performance optimization strategies
Supplement guidance
Results vary significantly by individual, and no specific outcomes are promised or guaranteed.
While our team provides guidance based on experience, data, and professional judgment, your results depend on many factors outside of our control, including your consistency, adherence to recommendations, individual physiology, and lifestyle.
By participating in our programs, you acknowledge that:
You are responsible for following recommendations provided
You are responsible for your personal health decisions and actions
To the fullest extent permitted by law, SPM is not responsible or liable for the success or failure of outcomes related to participation in our programs.
5. Supplements & Fulfillment Policy
Supplement recommendations are for general wellness support only and are not intended to diagnose, treat, cure, or prevent disease.
Supplements may be fulfilled in multi-month (e.g., 90-day) increments, especially for longer-term programs.
All supplements are custom-selected and ordered per client.
All supplement and product sales are final.
Due to health and safety regulations:
Products cannot be returned
No refunds will be issued once products are ordered or shipped
No refunds or credits will be given for unused supplements, even if a program is canceled early
You are not required to continue using supplements after cancellation, but you remain financially responsible for products already fulfilled.
6. Communication & Virtual Services
Services may be delivered via phone, video, or secure messaging platforms.
You acknowledge:
Electronic communication is not guaranteed to be fully secure
Technical issues or delays may occur
Communications may become part of your client record
Do not use these channels for emergencies.
7. Client Responsibilities
You agree to:
Provide accurate and complete information
Maintain updated contact and billing details
Keep your account login secure
Follow program guidance to the best of your ability
You are responsible for your health decisions and outcomes.
8. Payments, Subscriptions, & Billing
All payments are processed securely via Stripe.
Services may include:
One-time purchases
Recurring subscriptions (monthly, quarterly, annual)
Payment plans (including fixed or variable schedules)
By enrolling, you authorize SPM to automatically charge your payment method according to the billing terms presented at checkout.
Failure to complete payments may result in suspension or termination of services.
9. Refunds, Cancellations, & Subscriptions
Refund Requests
Fees for consultations and services are non-refundable once delivered
Refund requests must be submitted in writing using the official SPM Refund Request Form
Please allow up to 10 business days for review and processing
Approved refunds will be issued to the original payment method
Supplements & Lab Orders
Supplements are non-refundable once ordered or shipped
Lab orders are non-refundable once submitted
Subscriptions & Program Cancellations
Cancellation requests must be submitted at least 10 business days prior to the next billing date
All cancellations must be submitted via the official SPM Cancellation Form
Requests via text, email, or verbal communication are not considered valid
Programs Longer Than 3 Months
For programs lasting more than three (3) months, you are required to complete a final consultation with a licensed provider or designated team member prior to cancellation processing
Failure to complete this step may delay cancellation and apply charges to the next billing cycle
Billing Timing
Cancellations submitted less than 10 business days before the next billing date will apply to the following billing cycle
No partial refunds are issued for active billing periods
10. Consultation Scheduling & Rescheduling
At least 24 hours’ notice is required to reschedule appointments
Missed appointments or late cancellations may result in forfeiture of the session without refund
11. Disclosure & Affiliate Relationships
SPM may recommend third-party products or services
In some cases, SPM may receive affiliate compensation or referral fees
You are under no obligation to purchase recommended products
SPM may also work with affiliates who refer clients. These relationships do not affect pricing or the quality of services provided.
12. Limitation of Liability
To the fullest extent permitted by law:
You assume all risks associated with participation in our programs and use of our services
SPM is not liable for outcomes related to: Coaching recommendations Supplement use Third-party services or providers
SPM makes no guarantees regarding results or outcomes.
SPM’s total liability, if any, is limited to the amount paid for the specific service giving rise to the claim.
13. Governing Law & Dispute Resolution
These policies are governed by the laws of the State of Arizona, without regard to conflict of law principles.
You agree to first attempt to resolve any dispute by contacting [email protected].
Any disputes that cannot be resolved informally will be resolved through binding arbitration in Arizona, unless otherwise required by law.
To the fullest extent permitted by law, you agree that any dispute will be handled on an individual basis only and not as part of a class, consolidated, or representative action.
